Prescription Medications:

If your student needs to take prescription medication during the school day, please fill out the Medication Permission Form provided below. All prescription medication must be in a properly labeled prescription bottle from a pharmacy that includes the student's name, physician, instructions for administration, and is unexpired. A healthcare provider signature is required on the Medication Permission Form for the administration of prescription medications.

Non-prescription Medications:

If your student needs to take over-the-counter medication during the school day, please fill out the Medication Permission Form provided below. Non-prescription medication must be unexpired and received in the original container. Non-prescription medications will not be administered in doses higher than the label recommendations for age/weight without a physician's order. A healthcare provider signature is required on the Medication Permission Form for the administration of all medications.
