You must use this form to submit your technology inventory for 2005-06.
If you do not have any changes or additions then you must choose NONE under Equipment Type #1 and then check off all the options at the bottom of this form. When you complete that choose SEND to send your form. You will NOT be able to check out for the end of the year if I do not get a form....

Does not apply to me: I am not responsible for any kind of technology equipment (do not have to check any additional options)

 Email: I will move all the email correspondence that I want to keep to my archive directory - c:\mail and then copy that to my directory on the server.

 File Copy: I understand that I have to move all of my important files that I want to keep to my directory on the file server.

 Clean Equipment: I have cleaned all of the technology equipment that is my responsibility - I have dusted and wiped it all clean.

Virus Checked: I have downloaded the current virus definitions and run a check on all of the computers that are my responsibility.

 X-Clean: I have run the X-Clean program on all of the computers that are my responsibility.
